Enrollment Forms
We will be pre-enrolling all of our students for the 2022-2023 School Year!
New students enrolling at Terral Public School will still be required to submit necessary paperwork including copies of birth certificate, shot records, and social security card.
Students reenrolling will be required to submit an updated shot records.
We will have enrollment forms and additional forms pre-printed at the office to fill out.
IN ADDITION, PLEASE PRINT, COMPLETE, AND RETURN THE FOLLOWING FORMS IF APPLICABLE:
Chromebook Handbook (Grade 2-8)
OSSAA Physical Form (All athletes in grades 3-8)
Sudden Cardiac Arrest Form (All athletes in grades 3-8)
Concussion and Head Injury Form (All athletes in grades 3-8)
